Treatment Release Authorization and Financial Responsibility Forms

These standard forms for authorization to provide veterinary treatment(s) as well as your acknowledgement to be financially responsible for the professional services, medications and supplies provided your animal(s) are to be signed and dated prior to provision of any professional services or treatments. Absentee owners are requested to designate an agent or manager to act on their behalf. These forms will be provided at the time of your animal\'s admission to the clinic or facility.
